Instructions:  Please fill out the above form to send us your weekly recognition numbers.  This information should be submitted no later than midnight on Saturday in order to be included in Monday morning's recognition! 

Notes:

**Don't count guests twice.  If you had a product appointment with 6 people, one with 4 people and 1 with 1 person, you have a class, a group facial and a facial.  Don't count a group facial as a facial and then all those people within the group facial as individual facials.  Each appointment should be designated only once under one of those categories.  Then count ALL faces that tried the product for the week in your "Total Faces Completed".  In the example above, that would be 11.

**A Basic must include cleanser, moisturizer and foundation to be considered a "Basic".

**New Bookings are bookings you created in the last week.  Not bookings you held, bookings you booked!  These might include bookings over the phone or bookings you got from other appointments.
